Sports Massage is a deep massage designed to release tight muscles, ‘iron-out’ knots in muscles and enhance sports performance by optimising muscle function. It is NOT a relaxing massage, rather more a therapeutic remedial massage and it is very effective in increasing blood flow and promoting healing. It can be quite uncomfortable if the muscles are tight and you need to clearly communicate the pressure you need to your therapist: everyone feels things differently- and your therapist appreciates this and can adapt accordingly. It is not limited to sports people or those who play the occasional round of golf but want to avoid injury – anyone can benefit from a deep massage to feel freer and help correct postural habits. They all use hands-on techniques to mobilise joints and stretch and relax muscles to enable your body to function optimally. Osteopaths and chiropractors have joined forces in one training institute, recognising their similarities. Where they differ is that Osteopathy and Chiropractic tend to be more holistic, treating the surrounding areas, sometime even remote parts of the body which may be contributing to the problem area. Chiropractors tend to manipulate (crack) the joints more and treatment times tend to be shorter. Osteopaths may also choose to manipulate the joint but will also work on soft tissue which takes longer. Physiotherapists mobilise joints by moving them (as do osteopaths and some chiropractors) and don’t normally manipulate joints and they focus on rehabilitating muscles. Physio’s are also trained to work in most areas of a hospital including ITU but those who specialise in MSK (muscles and joints) in the private sector are very good at having a research-based, structured exercise regime to support the treatment. At About Backs & Bones Osteopathy and Physiotherapy are treatments for painful muscles, joints and ligaments which usually have become strained, often through injury or problems with posture. Sometimes there may be an underlying structural issue which makes this more likely, or a disease process which causes pain. Whatever the reason, we assess you carefully to determine whether we can help alleviate symptoms. Are there any side-effects and how safe is it? Most people feel more flexible, free, looser and relaxed following treatment and may have instant relief of pain. Sometimes a mild aggravation is experienced and for 24-48 hours you may feel a little more discomfort. This is normal with any musculoskeletal treatment eg massage or exercise. Ensuring you have had enough to eat and drink before your appointment and the use of ICE when recommended minimises any side effects . Compared with the, sometimes quite serious side effects from some medications, these minor inconveniences are not usually considered a problem. We become ingrained throughout our lives in adopting certain postures. After a while, it can become very difficult to change as muscles and joints tighten. Osteopathy and massage stretches tight muscles and helps you to strengthen those which have weakened; relieving pain and helping you naturally achieve a more efficient working posture. About Backs & Bones Work & HobbiesSedentary Work We work more efficiently and with higher moral when we feel good at work and being comfortable is paramount. It isn’t always necessarily for expensive ergonomic assessments and state of the art furniture; the first step is to become aware of how you are sitting and ask “Do changes need to be made in order to achieve a posture which causes you minimal strain?” Why Change? Poor posture causes tiredness as your body uses energy to bring you back into line over your centre of gravity. We understand the need to return to work quickly and have minimal time off and this needs to be balanced against the risk of further damage, or at least delayed healing. It’s a team approach –we patch you up, identify other causes which may have contributed to your-injury and give you the tools to manage yourself –but we are on-hand should you need us again or wish on-going maintenance treatment. let’s talk about DrivingMost of us cope well with the unnatural strain of sitting in one position in a vibrating machine- but if you drive long distances, especially under stress or in a vehicle not suitable for your body size or shape, you may soon find yourself with neck, shoulder, upper or lower back pain –even extending into your arms or legs. . Women are particularly susceptible to injury are cars are designed around male test dummies and the presence of breast make the tests invalid as the seatbelt doesn’t fit correctly; As a woman you are 46% more likely to be seriously injured in a car than a man (Invisible Women, Caroline Criado Perez). Often the way we sit can have a profound effect and when treating you for driving-related strain, we firstly examine the cause of your particular problem. Next we give you some simple tests so you can check out whether you and your car are compatible. These are particularly useful if you are thinking of changing your vehicle –but you can often alter your seat enough for your car to fit you better. We then help you to become aware of your own posture : after all –your muscles don’t lie! If they are tight, there is often a good reason and being aware of this can help you minimise the risks. And when we have helped you feel so much more comfortable after our treatment –we help you to stay that way –with easy stretches you can even do in the car (just stop the car first!). . let’s talk about HorseridingWe all bemoan the fact that we should be riding better, should be achieving better dressage marks, should be able to avoid falling off in the ménage! So what could we do to improve matters? Being balanced is integral in enabling our horses to move freely – we know this – we spend fortunes getting them in shape and seeing the horse osteopath/chiropractor/physiotherapist. And then what do we do? Get right back on board not realising it may be us that are unbalanced and contributing to our horses back/neck/shoulder/leg problems. We usually concede that we could ride better and of-course good quality riding tuition will help us achieve our potential and become more balanced, but if you have been told that you appear restricted or stiff –maybe it’s time to address that? At About Backs & Bones, Paula, one of our osteopaths, understands the problems riders face and knows exactly how to address them. If your body is built to allow the movement, osteopathy will coax it into complying. If your particular shape does not allow it –we are all capable of different ranges of movement –we can look at ways of compensating. Often we are stiff in our shoulders – sometimes a legacy of our day job in front of a computer or driving. This restricts the amount of feeling we have down the rein and often causes the horse to lean or fight against us. Or we may have back problems, common amongst those who work with horses- not surprising when we lug horse feed, hay bales and water all day! We may be stiff in our hips, pelvis or low back, meaning we cannot follow the horse’s movement with as much fluidity as we would like, restricting their paces. We keep treatments to a minimum as we know finances for the horse come first! This is still effective as long as you do the exercises prescribed! If good riding tuition is needed –we can link with the experts; recommending people we trust. If your own instructor want to talk to us, or jot down the problems being experienced, we will work specifically on those, communicating with both of you if needs be. Watch your postureGet a friend to video how you stand and walk – but don’t be upset if you’re not the cat-walk model you thought you were! We recognise each other at a distance by our stance and gait so if you want to be identified positively, start to stand and sit in a relaxed but upright posture, with your weight centrally balanced –minimising the stress on your body. Rest when you feel tiredListen to your body –and be truthful! If you genuinely need the rest, then that’s fine – there are no prizes for being a martyr and you may pay for it later. The joints of the lower back and pelvis need to be able to move to accommodate the changes in your body in pregnancy and during the birth, so preparing beforehand with some treatment makes sense. Pregnant women are more vulnerable to injury, thanks to those pregnancy hormones, especially in your back, pelvis, leg hips or groin. At a time when painkillers need to be kept to a minimum, you may benefit significantly from osteopathy. Back pain after delivery is common and is usually easily treated and can prevent a lifetime of ongoing discomfort, so seek care earlier rather than suffering, when it can be resolved more easily. For your baby Being born seems like a traumatic journey and the effect on a baby’s soft skull and skeleton is not fully understood. However, particularly after a long labour, a forceps or ventouse delivery or a very quick birth and even after a caesarean section, babies can sometimes seem fractious and irritable, which can have an effect on your early experiences with your baby. This is where we aim to help: The evidence for effective treatment for babies with osteopathy or cranio-sacral therapy is classed as inconclusive as it is impossible to have an ethical research model for a baby. We can only describe how babies react to our treatment: Generally they appear more settled and calm and usually parents report that this continues at home. We provide 2/3 treatments, monitoring any changes and will only continue treatment if we all agree that your baby seems to be benefitting. Parents bring their babies to us if they are excessively irritable and unsettled, have problems feeding or have a flattened head – see testimonials. For children Most knocks and bumps in childhood are not a problem but sometimes there are anxieties with the way children walk or with limps, even if they are not painful, but may be indicative of more serious underlying problems. The children we treat usually experience pain due to sports activities where their enthusiasm gets them into trouble! Or the opposite may be true and they may have postural strain especially linked to excessive screen use or heavy school bags. You may simply need a few tips, such as realising how important a good warm-up and warm-down is to minimise muscle, tendon and ligament strains, Your training footwear may be causing or contributing to your pain so a lower limb biomechanical assessment may be required, or you are just experiencing muscle tightness and need massage and specialist stretches to help. Tendon StrainsTendons are the tissue where the muscle belly narrows and changes in consistency to become more ‘stringy’, and less elastic and then attaches to a bone. They are subject to the same strains that muscle is, as they are, after all, at the ends of the muscle. They do not have such a good blood supply as the muscle, however, and so take longer to heal. The treatment is similar to muscle strains. Muscle StrainsMuscles become strained when they are under stress and tears occur when they are being asked to contract further. This may be because the load may be beyond their capabilities, or because they were already tight to begin with. This is why a good training regime is important to build muscle which cam cope with increasing amounts of stress, and why effective warm-ups and warm-downs are essential to ensure the muscle is not already tight prior to exercise. Once a muscle is damaged it needs a good blood supply to bring nutrients and remove waste products efficiently. It also needs to heal with the minimal amount of scar tissue to minimise further injury. The joints which the muscles surround need to be able to move freely to reduce strain on the muscles and if the skeleton is symmetrical, this also reduces needless strain. Treatment of both the muscles and joints ensuring symmetry and balance in the body reduces strain on muscles. Soft tissue massage, joint mobilisation and re-alignment can be provided by our massage therapists and osteopaths or physiotherapist. The emphasis then is on effective stretching and good warm-up and warm-down practices, which we will ensure is tailor-made to you. Ligament SprainsLigaments are connective tissue, often stringy’ similar to tendons but are less elastic and so prone to injury. Their function is to support joints in their optimal position and when they become damaged, other structures forming the joint also become vulnerable to damage. They do not have a good blood supply which means their recovery time is even slower than a fracture, and for some sprains, a hard cast is necessary to keep the joint supported to prevent further damage whilst the ligaments heal. Osteopathy and massage is useful whilst the healing is taking place as it minimises the pressure on the affected joint by ensuring surrounding joints are moving and functioning freely; taking the strain and compensating for the injury. It also reduces pain and helps disperse swelling by working on the soft tissue, which also promotes symmetry so the joint can heal in an optimal position. The surrounding muscles can often be trained to support the joint thus relieving the strain on the ligaments and for some individuals who are hypermobile, this may be the mainstay of their treatment. Shin Splints and Stress FracturesShin splints and stress fractures occur when a muscle is so loaded, instead of tearing in the muscle belly, it tears where the tendon attaches to the bone (shin splint). Sometimes this causes the bone to split on its outer edge (stress fracture) and because the muscle is constantly pulling on that same area as we walk or run, it does not get chance to heal effectively. Osteopathy and massage works on the muscles directly to reduce tension and promote a good blood supply to aid healing, whilst also ensuring the surrounding joints are mobile, reducing strain on the affected muscles. Achilles Tendonitis and Severs DiseaseThe calf muscles are powerful muscles which propel us forward in walking or running, or upward in jumping. They have to perform strongly and often suddenly which makes them more vulnerable to strain. The Achilles tendon is the weak point, as is the point at which it attaches to the heel bone and tendonitis here is common (see muscle strains for more detail and treatment). Severs disease occurs when the Achilles tendon pulls hard regularly on the heel bone so that a bony lump eventually forms which is painful. Osgood-Schlatters DiseaseThis is the same as Severs Disease (see above) but occurs just below the knee where the patella tendon inserts onto the tibial tuberosity, which then enlarges and becomes painful. In your appointment you will be asked details of your pain and will be observes in a physical assessment where we may ask you to undress (you may bring a chaperone if you wish), After a hand-on examination and possibly orthopaedic tests, a working diagnosis is made and a treatment plan discussed with you. Treatment starts within the same session with your consent, using a range of techniques with the aim of increasing your range of pain-free movement. Most osteopathy is surprisingly painless and we always work within your comfort zone for any bits which aren’t! Exercises are normally prescribed which greatly shortens the amount of treatment you will need and helps prevent recurrence. As your pain decreases, your treatments are spaced out until you are pain-free or managing well at home. Sometimes your pain is due to another disease process or medical condition and osteopathy may not be the first choice of treatment. We are trained to recognise this and can refer you to more appropriate treatment with your approval. About Backs & Bones helping you be the best you can beAbout Physiotherapy What is the difference between a physiotherapist, an osteopath and a chiropractor? This is an interesting question and one which could be debated forever! Paula trained as a chiropractor then swapped course to become an osteopath and has worked and trained with physiotherapists and has had this discussion many times… Broadly speaking, physio’s are trained for many hospital in-patient conditions and may choose, as Kasia has, to specialise in ‘MSK’ (musculoskeletal) disorders. Osteopaths and chiropractors are more similar to each other and are trained purely in ‘MSK’ . The treatment differ in that we use different techniques to manipulate and some osteopaths will also use massage in their treatment more than chiropractors generally do. However osteopaths, chiropractors and physiotherapists all attend each others post-grad courses so the overlap between us all blurs. Basically we can all treat you- just find someone you like…preferably at About Backs & Bones!! let’s meet our Fully Qualified Osteopaths Paula Paula started her working life as nurse, specialising in male surgical nursing and then trained as a midwife, qualifying from Guy and St Thomas’ hospitals and working at University College Hospital in London. After moving back to the Midlands, settling in Derby and having a family of her own, she began training to become a chiropractor and after 4 years went on to train as an osteopath. Her final dissertation was looking at the pain experienced at work which she finds particularly interesting. Since qualifying Paula has undertaken a course in cranial osteopathy which she use alongside structural osteopathy as needed. She has also completed the British Medical Acupuncture Society course, further broadening her skills and she has also built on her midwifery knowledge by attending several courses in osteopathy, physiotherapy and chiropractic specifically for pregnant women and their babies. She has also assisted in teaching at the London School of Osteopathy when her work schedule allows and has also delivered Manual Handling and Health and Safety presentations to health professionals. Paula attracts a wide range of patients, from pregnant women, babies and children, to sports people, especially horse-riders and those with work strain. Elderly people also often have regular treatment to remain more mobile and supple, as well as those simply looking to improve their health and keep well. Paula’s working life has always consisted of professional roles: therefore she has good experience and understanding of working alongside GP’s, dentists, midwives and health visitors, sports coaches and podiatrists, whilst maintaining confidentiality. She has a strong professional ethos underlying all of her work but at the same time, the sessions are relaxed and friendly which promotes good communication. Andy Like many in this profession Andy was introduced to osteopathy as a patient following a back injury many years ago. Inspired by the results of this drug-free yet highly beneficial form of medicine he first trained as a massage therapist. Ultimately, he gave up a long and successful career in the offshore Oil & Gas Exploration industry and embarked on 4 years of full-time study in osteopathy at the British College of Osteopathic Medicine (BCOM) in London (formerly the British College of Naturopathy and Osteopathy). Training at BCOM not only teaches osteopathy but also stresses the important roles of nutrition and psychology. When not in the clinic, Iain enjoys staying active through hiking, yoga, strength training and continually exploring new ways to optimize human movement and performance. let’s meet our Fully Qualified Physiotherapist Kasia Kasia is a qualified Registered Physiotherapist (HCPC Reg) with 9 years experience in treating sports injuries, neck, shoulder and back pain, different types of joint issues, soft tissues injuries and musculoskeletal disorders. In her practice she uses a combination of soft tissue therapy, mobilisations and exercises to maximise the effectiveness of treatment. She is also experienced in treating general complaints such as degenerative diseases and postural issues. As well as treating sports injuries and providing functional training, she is qualified to rehab pre and post-operatively: all too often there is disappointment after surgical procedures and sometimes this is due to lack of rehabilitation. Kasia, as a physiotherapist, is trained to provide pre and post operative training to maximise the effectiveness of surgery. Please do not confuse an appointment with an NHS physio with that of a private consultation -they are worlds apart and Kasia will treat you- not just give you exercises, even though we know they are beneficial! . let’s meet our Fully Qualified Massage TherapistsAs well as being an experienced qualified Sports Massage Therapist, Nina is also a Personal Trainer and has combined these skills to maximise effective rehabilitation. She started her therapy career under the guidance of Great Britain Rugby League Physiotherapist, Keith Warner, at the Hull Sports Medicine Centre where she gained valuable experience in treating and rehabilitating professional sports men and women as well as those suffering from occupational work stress, age related stiffness and pain and specific medical conditions. As well as more traditional sports massage techniques, Nina also has experience in taping and strapping and electrotherapy. Electrotherapy techniques typically consist of ultrasound and interferential, both of which are effective treatments and complement osteopathic and massage techniques. Taping and strapping can be particularly useful in aiding sportspeople to continue training whilst protecting an injury whilst it is healing. But sometimes a little extra help is needed and if appropriate, that’s when we turn to medical help. Sandy is a qualified doctor who specialises in musculoskeletal health and performs steroid injections guided by ultrasound. Many studies have shown this to be more accurate resulting in a better outcome: ie less pain. It is suitable for many joints throughout the body – once physical rehabilitation has been implemented. After the injection, you may feel so much better that you take no further action: but this is your window to building a good muscle balance to support the joint,- when you are not feeling in pain- and therefore making the money you have spent on the injection worthwhile as a longer-lasting effect is more likely (steroid injections cannot be repeated too often as they can weaken the ligaments in the joint). Hyaluronic acid injections are also an option, especially in an arthritic joint and the first consultation will ascertain which treatment, is any is suitable. This first consultation is over the phone- which saves you money and a physical assessment is performed when you attend for the injection. If it is deemed not to be affective for you, then just the consultation fee is charged.
